DATA SUBJECT APPLICATION FORM
In Article No 13 of the Law No 6698 on the Protection of Personal Data (hereinafter referred to as “KVKK“), personal data owners (hereinafter referred to as “Requester“), who are identified as the Data Subject, are granted the right to make certain requests regarding the processing of their personal data listed in Article No 11 of the Law by applying to the Data Controllers.
In the event that you wish to exercise these rights under KVKK, you are required to submit your requests to YEDIYOL TURIZM ANONIM ŞTI (hereinafter referred to as the “Company”), which is the Data Controller, by filling out this form. According to Paragraph No 1 of Article No 13 of KVKK and the provisions of the Communique on the Procedures and Principles of Application to the Data Controller (hereinafter referred to as the “Communique”), you may hand-deliver the completed Application Form, the wet signed version of the Form, together with the documents certifying your ID, to the address HASANPAŞA MAHALLESI SARAYARDI CADDESI CEMHAN DOĞAN İŞ MERKEZI NO 100 A İÇ KAPI NO 1 KADIKÖY/İSTANBUL or you may send it by Registered Mailor you may deliver it to our Company by Registered Electronic Mail (KEP) to the address “bozyaziturizm@hs06.kep.tr“.
In order to ensure the direct delivery of the application form to the relevant department, the phrase “Information Request within the scope of the Law on the Protection of Personal Data” should be placed on the envelope in the case of physical applications, and the same phrase should be placed on the subject line of the e-mail in the case of applications to be made via e-mail within the scope of one of the above-mentioned methods.
The applications to be made within the scope of Paragraph No 2 of Article No 4 of the Communiqué must be made in Turkish. Your application shall be examined immediately by our Company and shall be responded to within thirty days from the date of receipt of your application by our Company within the scope of the provisions of KVKK and the Communique, and the final decision shall be communicated to you in writing or in the electronic environment.
As a rule, our Company does not charge any fee in relation to the applications made, however, if the transaction requires an additional cost, fees determined by the Board may be charged. In the event that the information and documents submitted by you are incomplete or incomprehensible, you shall be contacted so that we can clarify your application.
The fields marked (*) must be filled in accordance with Article No 5 titled “Application Procedure” of the Communiqué on the Procedures and Principles of Application to the Data Controller, which was prepared based on Article No 13 (of Law No 6698 on the Protection of Personal Data) and Article No 22 (of Law No 6698 on the Protection of Personal Data), Paragraph No 1, Sub-paragraphs (e) and (g). If this field is empty in your application, your application shall not be taken into consideration.

This Application Form, which you have filled out, is issued in order to determine your relationship with our company and to respond to your personal data processed by our company, if any, in a complete and accurate manner and to respond to your application correctly and within the legal period. In order to avoid legal risks that may arise from unlawful and unfair data sharing and especially to ensure the security of your personal data, our Company reserves the right to require additional documents and information (copy of ID Card or Driver’s License etc.) for identification and authorization. Our company is not responsible for any problems that may arise during mailing to the address/e-mail. In the event that the information regarding your requests submitted within the scope of the form is not correct and up-to-date or an unauthorised application is made, our company shall not be liable for any claims arising from such incorrect information or unauthorised application.
